Medical Continuity Coordinator

**Job Description:**
We are seeking a Medical Continuity Coordinator to join our team at LifePoint Hospitals. As a Medical Continuity Coordinator, you will be responsible for assessing, evaluating, planning, organizing, and implementing activities to ensure the continuity and quality of patient care.
**Responsibilities:**
- Evaluate patients' medical history and current condition to develop individualized care plans.
- Assess patients' physical and emotional needs, and provide appropriate interventions to promote recovery and well-being.
- Collaborate with other healthcare professionals to coordinate patient care and ensure seamless transitions between departments.
**Requirements:**
- Degree in Nursing from an accredited institution.
- Current RN license and CPR certification.
- Excellent communication and interpersonal skills.
**Estimated Salary:** $75,000 - $90,000 per year, depending on experience.
