Service Coordinator

Overview

The ARC Fresno Madera Counties is seeking a highly skilled Service Coordinator to join our team. This role will be responsible for providing case management services, counseling, intervention, and implementation of client plans to assist in developing, maintaining, increasing, and maximizing independent functioning.

Key Responsibilities
  • Coordinates Client Services: Coordinates all client services for assigned caseloads, ensuring that clients receive the necessary support and resources to achieve their goals.
  • Provides Consultation: Provides consultation as appropriate to assist program staff and clients in achieving goals and objectives, promoting collaboration and teamwork.
  • Develops and Implements ISP's: Creates and implements Individual Service Plans (ISP's) and Individual Habilitation Component (IHC) reports, outlining clear goals and objectives for each client.
  • Counsels and Intervenes: Provides individual and group information in the areas of vocational, personal, and social adjustment, helping clients to overcome challenges and achieve independence.
Requirements
  • Education: Associate's degree from an accredited college or university;
  • Experience: One to two years related experience and/or training, or equivalent combination of education and experience.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with clients, staff, and external partners.
  • Salary: $45,000 - $55,000 per year, depending on experience.
