Key Responsibilities:
Organize resident schedules for necessary admissions, annual, quarterly, Medicare evaluations, or assessments due to changes in condition, ensuring effective communication with nursing and other departments.
Ensure adherence to Federal and State regulations concerning the completion and coordination of the comprehensive assessment (MDS).

Oversee MDS and care plan documentation for all residents, guaranteeing that records are complete, organized, chronological, and up-to-date.

Maintain proficiency in relevant software applications.
Facilitate and participate in regular Medicaid meetings to review current and pending residents.

Coordinate and attend all Interdisciplinary Team Meetings (ITTM) to gather insights, communicate updates, and maintain clinical records, including care plans.

Update care plans as necessary.
Provide the Director of Nursing, Administrator, and relevant staff with the Quality Indicators Report.
Ensure timely transmission of the MDS.
Stay informed about industry changes by attending external workshops and conferences.
Manage ancillary services, including collection and reporting to corporate billing personnel.

Qualifications:
Active Nursing License in good standing.
Experience in geriatric nursing.
Commitment to restorative care principles, along with strong professional skills in both assessment and hands-on care, as well as effective communication and organizational abilities.
Familiarity with nursing home regulations.
Capability to work independently while also being a collaborative team member.
Excellent organizational skills to manage multiple projects simultaneously.
