Job Summary

We are seeking a highly skilled Home Health Physical Therapist to join our team at Advocate Aurora Health. As a key member of our home health care team, you will be responsible for providing high-quality physical therapy services to patients in their homes, promoting optimal functional outcomes and improving quality of life.

Key Responsibilities
  • Conduct comprehensive physical therapy evaluations to identify patient needs and develop personalized plans of care.
  • Implement evidence-based interventions to address patient goals and objectives, including exercises, modalities, and education.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Monitor patient progress, adjust treatment plans as needed, and provide regular updates to physicians and caregivers.
  • Manage patient caseloads, prioritize tasks, and maintain accurate records and documentation.
  • Participate in quality improvement initiatives, contribute to departmental goals, and maintain current knowledge of best practices in physical therapy.
Requirements
  • Current Physical Therapist license in the state of Wisconsin.
  • Basic Life Support (BLS) certification from the American Heart Association.
  • Valid driver's license and reliable transportation.
  • 3+ years of experience in physical therapy, preferably in home health care or a related setting.
  • Excellent communication, problem-solving, and time management skills.
  • Ability to work independently and as part of a team, with a focus on patient-centered care.
