Job Summary:

We are looking for a skilled and compassionate Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Physical Therapist with Luxe Home Health, you will conduct thorough assessments, develop and implement treatment plans, and provide excellent patient care.

Responsibilities:
  • Conduct thorough and precise assessments of patient condition and functional abilities
  • Develop, coordinate, and revise treatment plans based on physician recommendations
  • Provide care in accordance with physician orders, patient condition, accepted standard of practice, and agency policies
  • Follow patients for weekly visits per patient needs and physician orders
  • Complete paperwork, prepare accurate and timely reports, and required documentation in all applicable systems per agency standards and protocols
Requirements:
  • Valid PT License in State of MO
  • MO Drivers License with the ability to drive within the given territory
  • Valid auto insurance
  • Excellent charting skills (Axxess - preferred)
  • Home Health experience preferred
