Job Title: Home Health Physical Therapist

We are seeking a highly skilled and compassionate Home Health Physical Therapist to join our team at Advocate Aurora Health. As a Home Health Physical Therapist, you will play a vital role in providing high-quality patient care and rehabilitation services to patients in their homes.

Job Summary:

The Home Health Physical Therapist will conduct thorough evaluations of patients, develop and implement individualized plans of care, and provide ongoing assessment and treatment to ensure optimal patient outcomes. You will work closely with our interdisciplinary team to coordinate care and ensure seamless transitions between settings.

Responsibilities:
  • Conduct comprehensive evaluations of patients to identify physical therapy needs and develop individualized plans of care.
  • Provide high-quality physical therapy services to patients in their homes, including exercises, modalities, and education.
  • Collaborate with patients, families, and healthcare providers to ensure effective communication and coordination of care.
  • Monitor patient progress and adjust treatment plans as needed to ensure optimal outcomes.
  • Document patient information and treatment plans accurately and thoroughly.
  • Participate in ongoing education and professional development to stay current with best practices and evidence-based care.
Requirements:
  • Current Physical Therapist license in the state of Wisconsin.
  • Basic Life Support (BLS) certification.
  • Valid driver's license and reliable transportation.
  • 3+ years of experience in home health or similar setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
