Job Overview

As a Clinical Care Coordinator RN, you will be part of an interdisciplinary care team responsible for outreaching and scheduling member face-to-face visits in homes, dialysis clinics, and physician offices.

About the Role
This field-based position meets with members in their homes and facilities, traveling at least 75% of the time. The territory primarily covers the greater North Houston TX area. You will work closely with nephrology practices to support improvement in members' health outcomes. Your primary focus will be on providing quality care through close communication with members, identifying medical, behavioral, and social determinants of health gaps or barriers in treatment plans, and connecting them to community resources.

Responsibilities
- Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
- Travel to complete in-home and in-facility visits with members
- Participate in care planning in facilities and provider offices
- Use nursing assessment skills to identify medical, behavioral, and social determinates of health gaps or barriers in treatment plans
- Provide condition education, conduct medication reviews, assist with advance planning, and connect to community resources
- Demonstrate ability to establish trusting relationships with members and caregivers and identify innovative strategies to improve and maintain engagement
- Participate in interactions with interdisciplinary teams, treating providers, and community resources
- Complete in-patient and post-discharge visits to ensure discharged members receive necessary services and resources to focus on reducing readmissions
- Documenting and tracking findings; solid computer and software navigation skills are critical
- Complete and maintain field-based safety trainings

Requirements
- Current, unrestricted RN license in the state of Texas
- Ability to get licensed in other states as needed
- 3+ years of RN experience
- Renal, Chronic Kidney Disease, or Dialysis Care experience as a main focus of your job
- Computer proficiency, including strong data entry, utilizing MS Office (Word, Excel, PowerPoint, and Outlook), and the ability to type and talk at the same time while navigating a Windows environment
- Reside in a location that can receive a high-speed internet connection or can leverage existing high-speed internet service
