Job Description:
We're seeking a RN Healthcare Manager to join our team in Houston, TX. As a member of the Optum Care Delivery team, you'll be an integral part of our vision to make healthcare better for everyone. You'll work alongside talented peers in a collaborative environment that values diversity and inclusion while driving towards the Quadruple Aim.
**Responsibilities:**
* Work with providers and clinic staff to identify patients at high risk of health conditions.
* Support longitudinal care of patients with chronic care conditions by performing assessments, medication reconciliations, motivational interviewing, self-management goal setting, patient education, and referrals to appropriate agencies and resources.
* Assist with transitioning patients from inpatient to outpatient settings, including assessment of transitional needs, medication reconciliation, establishing and reviewing contingency plans, providing patient education, and assisting with post-discharge needs such as prescriptions, transportation, DME, and appointments.
* Coordinate with providers to establish or update plans of care, perform accurate and timely documentation in the electronic medical record, participate in daily huddles and Patient Care Coordination meetings, prepare reports for weekly meetings, maintain continued competence in nursing practice and knowledge of current evidence-based practices, and perform all other related duties as assigned.
Requirements:
* Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience as an RN.
* Registered Nurse with an active and unrestricted license to practice in the state of employment.
* Current BLS certification.
* 2+ years of experience in a physician's office, clinical, or hospital setting.
* Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD.
* Proficient computer skills to work efficiently with electronic medical records.
* Proven excellent verbal and written skills, solid interpersonal skills, ability to interact productively with individuals and multidisciplinary teams, and excellent organizational and prioritization skills.
Preferred Qualifications:
* Experience related to patient education and/or motivational interviewing skills and self-management goal setting.
* Fluent written and verbal skills in English and Spanish.
Work Environment:
The ideal candidate will have the ability to lift up to 35 pounds, push or pull heavy objects using up to 35 pounds of force, sit for extended periods of time, stand for extended periods of time, use fine motor skills to operate office equipment and/or machinery, receive and comprehend instructions verbally and/or in writing, and use logical reasoning for simple and complex problem-solving.
