Family Partner - ACCS

Eliot's Adult Community Clinical Services (ACCS) program is seeking a dedicated and compassionate Family Partner to join our team. As a Family Partner, you will work closely with families and individuals to provide support, guidance, and advocacy in navigating the mental health system.

Responsibilities:
  • Provide in-home and community support to youth and adults and their families during behavioral health emergencies, with a focus on assisting parents and caregivers.
  • Work with the ACCS team and families to provide in-person and telephonic support, referrals, and linkage to other services.
  • Offer support, modeling, and skill development to parents, caregivers, and family members, acting as a liaison and advocate between families and the mental health or educational system.
  • Ensure that all services provided are culturally and linguistically appropriate, accessing and utilizing resources to meet the needs of individuals and families.
  • Assist parents and caregivers in developing advocacy skills for their child or adult family member, including participation in meetings in the school and community.
  • Support families in developing a strengths-based treatment plan and safety plan, in conjunction with ACCS clinical recommendations, including crisis intervention techniques to help diffuse situations and connect individuals and families to needed supportive services.
  • Work with families and ACCS to identify and apply for needed local, state, and federal resources, such as benefits, housing, and employment support.
Schedule:

Monday to Friday, 8am to 4pm
