Job Summary

The Field Intake Nurse is a critical role at Centers Plan for Healthy Living, responsible for assessing a member's eligibility for the Managed Long Term Care program. This position requires a comprehensive understanding of CPHL's program and services, as well as excellent communication skills to effectively convey member benefits.

Primary Responsibilities
  • Conduct face-to-face comprehensive assessments with potential members in their place of residence, utilizing the NYSDOH-approved assessment tool (UASNY).
  • Complete all necessary documentation to meet CPHL standards, utilizing CPHL's database of record.
  • Communicate plan benefits clearly and effectively to prospects and referral sources.
  • Submit thorough and high-quality documentation in a timely manner.
  • Recognize quality of care issues and escalate them appropriately, communicating effectively with coordinators and supervisors as needed.
Requirements
  • Graduate of a 2-4 year accredited nursing program.
  • Active and unrestricted NYS RN license.
  • Strong written and verbal communication skills.
  • Proficient in the use of Microsoft Office Suite tools.
  • Ability to travel within assigned area.
