Job Title: Field Registered Nurse Assessor

Maximus is seeking a Field Registered Nurse Assessor to join our team in Bronx, NY. As a Field Registered Nurse Assessor, you will be responsible for conducting home assessments and providing quality care to vulnerable populations.

About the Role:

The Field Registered Nurse Assessor will work closely with the clinical leadership team to conduct home assessments in the Bronx. This role requires attending and conducting assessments in consumer homes, facilities, or through a telehealth modality, depending on operational need.

Key Responsibilities:
  • Travel: Clustered field visits limited to one borough or geographical area
  • Assessments: Perform in-home, in-facility, or telehealth assessments that includes evaluation of individual's health status, strengths, care needs, and preferences using the UAS-NY assessment tool within the prescribed timeframes of the assessments.
  • Partnership: Establish and maintain positive relationships with consumers, caregivers, and facility staff.
  • Accuracy: Regularly review medical documentation, reporting any concerns, conflicting information, or issues that may arise during the evaluation process.

Requirements:
  • Current Registered Nurse (RN) license valid in the state of practice is required
  • Bachelor's degree and 5 years of experience or equivalent combination of education and experience required
  • Minimum of 5 years of clinical experience
  • Minimum 1 year supervisory experience preferred
  • 2+ years RN experience in-home care.
  • Active and unrestricted RN license in the state of New York.
  • Bachelors' Degree in health, social services, or communications.
  • Ability to travel locally.
  • Reliable form of transportation may include ride-sharing options.
Preferred Qualifications:
  • Prior experience using the UAS-NY Community assessment tool.
Home Office Requirements:
  • Maximus provides company-issued cellphone.

  • Reliable high-speed internet service with 100 Mbps download speed
  • Private and secure home office workspace.

Annual Base Pay Minimum for this Position: $107,000.00

Annual Base Pay Maximum for this Position: $118,000.00
