About the Role

We are seeking a highly skilled Care Manager I-Waiver to join our team at Alliance. As a Care Manager I-Waiver, you will play a critical role in ensuring that individuals and families with special health care needs receive integrated, whole-person-centered care management.

Key Responsibilities
  • Complete comprehensive assessments and develop Plans of Care to address the unique needs of our members.
  • Collaborate with community systems and stakeholders to provide coordinated care and support.
  • Engage with providers to identify barriers to service delivery and work towards individualized resolution.
  • Document all applicable member updates and activities, ensuring compliance with organizational procedures.
  • Proactively respond to member transitions and ensure a smooth transition without lapse in care.
Requirements
  • Bachelor's degree in a Human Services field and two years of post-bachelor's degree experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management.
  • Or Bachelor's degree in a Non-Human Services field and four years of post-bachelor's degree experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management.
  • Or Master's Degree in a Human Services field and one year of post-graduate degree experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management.
  • Or Fully or Provisionally Licensed in the State of North Carolina as a LCSW, LCMHC, LPA, or LMFT and two years of LTSS and/or HCBS coordination, care delivery monitoring, and care management experience.
  • Or Licensed Registered Nurse (RN) in the State of North Carolina with four years of mh/dd/sa experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management.
Preferred Qualifications
  • NACCM, NADD-Specialist, and/or CBIS Certification.
