Nurse Case Manager

Mass General Brigham is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager. This is an exciting opportunity to make a meaningful difference in the lives of our patients and their families.
Job Summary:
The VNA RN Case Manager is responsible for providing high-quality, patient-centered care to individuals and families in their homes. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Responsibilities:
- Provide skilled care and case management to patients and families in accordance with the State Nurse Practice Act and accepted professional standards of practice.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Assess patient needs and develop individualized care plans to promote optimal health outcomes.
- Collaborate with interdisciplinary teams to ensure comprehensive care and address complex patient needs.
- Document patient information and progress in a timely and accurate manner.
Requirements:
- Graduate of an approved, accredited school of professional nursing required; BSN preferred.
- Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required.
- Current CPR certification required.
- Minimum of one (1) year Medical/Surgical nursing experience required; home care/hospice experience preferred.
- Demonstrated excellent verbal and written communication skills and strong interpersonal skills required.
- Demonstrated organizational and time management skills required.
- Current driver's license, a good driving record, and reliable transportation required.
