Nurse Case Manager

Mass General Brigham is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a VNA RN Case Manager. This is an exciting opportunity to provide high-quality care and case management to patients and families in the comfort of their own homes.
Job SummaryThe VNA RN Case Manager is responsible for providing skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice. This includes assessing patient needs, developing and implementing individualized care plans, and coordinating with healthcare teams to ensure seamless care transitions.
Responsibilities- Assess patient needs and develop individualized care plans
- Provide skilled care and case management to patients and families
- Coordinate with healthcare teams to ensure seamless care transitions
- Communicate effectively with patients, families, and healthcare teams
- Document patient information and care plans accurately and thoroughly
- Graduate of an approved, accredited school of professional nursing required; BSN preferred
- Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required
- Current CPR certification required
- Minimum of one (1) year Medical/Surgical nursing experience required; home care/hospice experience preferred
- Demonstrated excellent verbal and written communication skills and strong interpersonal skills required
- Demonstrated organizational and time management skills required
- Current driver's license, a good driving record, and reliable transportation required
