Nurse Practitioner

Mass General Brigham is seeking a skilled Registered Nurse (RN) to join our Home Health team in Hatfield, Massachusetts.
Job Summary:We are looking for a dedicated and compassionate Registered Nurse to provide high-quality patient care and case management services to patients and families in their homes. The successful candidate will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that promote optimal health outcomes.
Responsibilities:- Provide skilled nursing care and case management services to patients and families in their homes.
- Conduct comprehensive assessments of patients' physical, emotional, and social needs.
- Develop and implement individualized care plans that address patients' unique needs and goals.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Provide education and support to patients and families to promote health literacy and self-management skills.
- Document patient care and progress in a timely and accurate manner.
- Graduate of an approved, accredited school of professional nursing required; BSN preferred.
- Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required.
- Current CPR certification required.
- Minimum of one (1) year Medical/Surgical nursing experience required; home care/hospice experience preferred.
- Demonstrated excellent verbal and written communication skills and strong interpersonal skills required.
- Demonstrated organizational and time management skills required.
- Current driver's license, a good driving record, and reliable transportation required.
