Position Overview:
We are seeking a dedicated and experienced Pediatric Hospice & Palliative Care Manager to oversee the management and coordination of care services for pediatric patients and their families. This role is essential in ensuring that our young patients receive the highest quality of care during their hospice and palliative journeys.
Key Responsibilities:
- Lead the Pediatric Palliative and Hospice Programs under the guidance of the Clinical Director.
- Collaborate with the Director of Patient Care Services to strategically enhance the Pediatric program across the region.
- Participate in the formulation of policies, procedures, and workflows that align with the anticipated growth of the program.
- Establish and maintain strong referral pathways by partnering with vendors, hospitals, and healthcare facilities.
- Coordinate new patient referrals and ensure appropriate staff assignments for optimal patient care coverage.
- Facilitate interdisciplinary team meetings, ensuring compliance with care standards and providing team support.
Qualifications:
- A minimum of two years of nursing experience or social work practice in a home health, hospice, or palliative care environment within the last five years.
- Preferred supervisory experience of at least two years within the past seven years.
Licenses and Certifications:
- Current registered nurse license in the State of California; BSN or Master's in Social Work (LCSW) preferred.
- HPNA certification is a plus.
- Valid California Driver's License and automobile insurance meeting state requirements, if applicable.
Compensation:
The position offers a competitive salary range of $135K-$170K annually, along with comprehensive benefits and a supportive work environment.
