Per Diem Compensation:

Position Overview:

As an LPN, you will play a crucial role in our interdisciplinary team, overseeing the daily clinical care of each resident and sub-acute patient.

Our LPNs have a unique opportunity to gain extensive clinical experience while providing advanced care in a supportive environment.

Your responsibilities will include planning, developing, organizing, implementing, and evaluating resident care to ensure the highest quality and compliance with regulations.

A valid state LPN license is required.

Key Responsibilities:

LPN Responsibilities:
LPNs are responsible for administering medications and treatments to assigned residents as per physician orders, adhering to the five rights of medication administration.

Supervise and guide the personal care and nursing procedures performed by Nursing Assistants on the assigned team.

Assist Nursing Assistants with direct resident care as needed.

Report any unusual symptoms, changes in condition, daily needs, and progress of residents to the Unit Manager/Assistant Unit Manager or Charge Nurse Supervisor.

Follow the established Plan of Care for each resident and monitor the Nursing Assistants' implementation of the care plan.

Manage the receipt and storage of controlled substances, ensuring proper and safe handling of medications.

Document relevant information in care plans, Nursing Notes, Medical Administration Records, Treatment Records, and other required documentation.

Qualifications:

LPN Qualifications:
Must possess credentials as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a current state license.

Experience in a geriatric setting is preferred.

Ability to supervise and collaborate effectively with other staff members is essential.

Strong verbal and written communication skills are required.

A caring and respectful attitude towards residents is necessary.

Physical stamina for continuous activity is required.

Ability to perform tasks to established standards of excellence is essential.

This role involves regular interaction with residents, coworkers, visitors, and supervisors.

To ensure a safe working environment, the employee must be able to read, write, speak, and understand the English language at an intermediate or advanced level.
