Job Description

Hudson Headwaters Health Network is seeking a skilled LPN to join our team in a part-time capacity.

Job Summary:

The Licensed Practical Nurse provides age-appropriate, population-specific patient care in a professional, compassionate, and culturally sensitive manner. The LPN administers care to patients under the direction of the responsible Registered Nurse or medical provider.

Key Responsibilities:

  • Provide direct and indirect care that is safe, ethical, patient-centered, and evidence-based.
  • Collaborate with staff and other departments to continuously improve quality of care and the patient experience.
  • Utilize data to meet care coordination needs across settings in conjunction with the patient, family, and/or other care providers.
  • Protect patients and employees by adhering to infection control policies and protocols.
  • Prepare patients for encounters by following established protocols.
  • Demonstrate competence in clinical skill sets.
  • Reconcile medication lists and prepare prescription renewals.
  • Demonstrate complete, accurate, and timely documentation in the electronic medical record.
  • Respond to patient phone calls and engage providers or RNs appropriately.
  • Reinforce patient and family education plans that meet patient needs.
  • Perform ordered point-of-care testing, including specimen collection and proper documentation.
  • Assist with patient encounter flow and troubleshoot issues in collaboration with supervisors.
  • Provide guidance and mentor new and incumbent staff based on skill sets.
  • Attend meetings and serve on various organization committees.
  • Assist with inventory and receipt of medical supplies, pharmaceuticals, and equipment.
  • Complete health center daily, weekly, or monthly tasks as assigned.
  • Ensure operation of medical equipment by following manufacturer instructions, troubleshooting malfunctions, and requesting repairs.

Requirements:

  • Unrestricted current LPN license to practice nursing in New York State.
  • Must possess a certificate from an accredited school of nursing.
  • Minimum of 1 year of clinical experience preferred.
  • Ability to travel throughout the Network.
  • Current BLS certification.
  • Proficiency with Microsoft applications and electronic medical records.
  • Excellent verbal and written communication skills.
  • Self-directed, detail-oriented, motivated, and ability to work independently as well as in a team atmosphere.

Pay Rate: $28.00/hour.
