Position Overview:

Role Summary:
The Community Clinician will provide essential support to families, ensuring that youth receive the necessary services in a nurturing, community-based environment. This role emphasizes the importance of maintaining continuity of care and fostering positive relationships with key figures in the youth's life.

Key Responsibilities:

  • Deliver direct clinical support to caregivers and multidisciplinary teams for a caseload of 7-10 youth.
  • Organize and lead monthly team meetings involving youth, caregivers, and relevant support personnel.
  • Facilitate ongoing communication with casework agencies to assess family and child needs.
  • Provide counseling and coaching to caregivers on trauma-informed practices and the promotion of secure attachments.
  • Collaborate with the child and family team to create and adjust personalized care plans.
  • Utilize trauma-informed interventions and evidence-based models tailored to the needs of children and families.
  • Ensure timely documentation of all clinical activities, including progress notes and treatment plans.
  • Offer clinical consultation to case management staff regarding effective treatment approaches.
  • Advocate for youth in various settings, ensuring their needs are met.
  • Travel as necessary for in-person sessions with families.

Qualifications:

  • Master's Degree in social work or a related field.
  • LCSW or LCPC licensure preferred, but not mandatory.
  • Proficiency in Microsoft Office and familiarity with video conferencing tools.
  • Strong interpersonal, communication, and time management skills.
  • Valid driver's license and access to a reliable vehicle.
