Nurse Practitioner

We are seeking a highly skilled Registered Nurse to join our team at SB Clinical Practice Management. The successful candidate will provide high-quality individualized direct patient care, utilizing their skills and knowledge of the profession.
Responsibilities- Accurately collect clinical data on patients and demonstrate competency in performing clinical interventions and reporting abnormalities to Medical Providers.
- Triage patient phone calls and troubleshoot medication management issues.
- Demonstrate competency in answering phones, assembling charts, patient registration, making appointments, computer data entry, and retrieval of reports.
- Must safely perform all patient care responsibilities in accordance with professional standards and the organization's policies and procedures.
- Develop and organize a plan of care in consultation with treating physicians, as indicated within the various subspecialties of the Department.
- Must effectively determine and set priorities to meet patient needs.
- Provide proper and accurate documentation of patient care in the patient medical record.
- Coordinate any services or accommodations necessary to ensure effective communications between treating providers and patients who are hearing and/or visually impaired, and patients with language barriers.
- Ensure that patients receive timely and appropriate care within the delivery system and facilitate internal and external referrals.
- Administer medications, treatments, and contrast agents according to the specific orders of the ordering physician, and document in the patient record.
- Maintain infection control precautions and identify potential infection control problems.
- Provides in-service education to members of the interdisciplinary team within the Department.
- Participate in Quality Assurance activities within the Department.
- Participate in annual recertification training, compliance training, and HIPAA training, as well as all other mandatory in-service programs.
- Conform to policies and regulations governing patient rights and confidentiality of information.
- Perform all other related duties as assigned by management.
- Associate's degree in Nursing.
- Current registration to practice as a Licensed Registered Nurse in New York State.
- Basic Cardiac Life Support (BCLS) required.
- Must have excellent communication skills.
- Bachelor's degree in Nursing.
- Advanced Cardiovascular Life Support (ACLS) preferred.
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to communicate with patients, staff, and medical providers. The employee must be able to exchange accurate information in these situations. This position is active and requires the employee to constantly move about in the office to assist patients, staff, and medical providers. Any additional physical demands will be outlined and provided by management.
