About the Role:
This is a challenging and rewarding role for a skilled Registered Nurse (RN) to coordinate patient care for patients with cardiovascular conditions. The successful candidate will work closely with a multidisciplinary team to provide comprehensive care to patients throughout their treatment journey.
Key Responsibilities:
- Coordinate patient care plans in collaboration with physicians, nurses, and other healthcare professionals
- Develop and maintain accurate and up-to-date patient records
- Provide education and support to patients and their families on cardiovascular health and disease management
- Collaborate with the healthcare team to identify and address any barriers to care
Requirements:
- Current RN licensure in South Dakota
- 2+ years of experience as a registered nurse in a hospital setting
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
