Overview

Christian Horizons is seeking a skilled MDS Coordinator to join our team at Lewis Memorial Christian Village. As a key member of our nursing staff, you will play a vital role in ensuring the highest quality of care for our residents.

Responsibilities
  • Conduct thorough assessments of resident care needs and develop individualized care plans.
  • Schedule and facilitate resident care conferences to ensure seamless coordination among departments.
  • Interview residents and their families to gather information for the minimum data set (MDS) assessment.
  • Complete timely and accurate MDSs and CMIs for all residents.
  • Review medical records and other documentation to collect information for MDS records.
  • Evaluate resident care in relation to individualized needs, family involvement, and physician plans of care.
  • Direct and organize required resident assessments and documentation for Medicare, Medicaid, and private insurance reimbursement.
  • Identify and report problems in an objective manner, taking corrective action as needed.
  • Collaborate with the interdisciplinary team to provide high-quality care to all residents.
  • Foster a sense of self-respect and worth in each resident through kindness, understanding, and patience.
  • Provide exceptional customer service to residents, families, staff, and external customers.
  • Ensure nursing service compliance with federal, state, local, and corporate regulations.
  • Participate in continuing education programs to stay current with changes in the profession.
  • Develop and conduct in-service training classes to educate staff.
  • Maintain accurate records, following corporate policies and procedures.
  • Work cooperatively with residents, staff, volunteers, and families to ensure the best quality care.
  • Manage staff and resources effectively.
  • Follow all written and oral directions from supervisors.
  • Participate in all required in-services and activities.
  • Use proper equipment and techniques to ensure safety.
Qualifications
  • Graduate from an accredited school of nursing.
  • Current state license as an LPN or RN.
  • Clear understanding of MDS required.
  • 1 year experience with MDS preferred.
  • Able to follow and give written and oral directions.
  • Demonstrate dependability, cooperation, and interest in caring for the elderly.
  • Display leadership qualities, good communication skills, and a desire to continuously learn.
  • Possess the ability to communicate effectively and deal tactfully with personnel, residents, families, visitors, government agencies, and the general public.
