Job Summary

The MDS Coordinator is responsible for coordinating care plans in accordance with regulatory requirements. This role ensures that essential resources are available to patients and that patient care is delivered effectively and to a satisfactory standard.

Key Responsibilities
  • Coordinate the development and completion of the Care Center resident assessment.
  • Monitor and ensure care plans are adhered to, properly documented, and residents receive the best care in accordance with TJC, federal, state, and local guidelines.
  • Communicate with medical staff, other departments, and outside agencies while maintaining confidentiality.
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, filing, etc.
  • Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, guidelines, policies, and procedures.
  • Quarterly and annually review and revise the MDS and care plan.
  • Ensure that the care plan includes measurable objectives and timetables to meet the resident's medical, nursing, mental, and psychosocial needs.
  • Review complaints made by residents or their families regarding the resident assessment of care plan and document the grievance, actions taken, and current status of complaint.
  • Watch for any sign of abuse and report all concerns to appropriate managers, CEO, or agencies.
  • Develops, implement, and maintain an ongoing quality assurance program for the resident assessment/care plans.
  • Assist the resident and Discharge Planning Coordinator in completing the care plan portion of the resident's discharge plan.
  • Participate in surveys (inspections) made by TJC and other authorized government agencies.
  • Act as Chairperson of the Interdisciplinary Care Plan Team and develop a comprehensive resident assessment and care plan for each resident.
  • Participate in discharge planning as necessary.
  • Develops and maintains good relations with other departments, providers, facilities, and agencies that may be involved in the resident assessment/care plan functions.
Requirements

The ideal candidate will have a high school diploma and completion of a minimum Practical Nurse Training course of study. A Registered Nurse level is preferred. The candidate should have management skills with experience in planning, organizing, implementing, facilitating, interviewing, counseling, and verbal and written communications. A Georgia Licensed Practical Nurse license or Registered Nurse License and Basic Life Support CPR certification is required.
