Position Overview:
Millennium Physician Group is in search of a dedicated and enthusiastic Healthcare Case Manager to become part of our ACO team. This full-time role operates Monday through Friday from 8 AM to 5 PM.
Key Responsibilities:
The Healthcare Case Manager plays a vital role in providing direct services that support patients in achieving optimal health outcomes. This position emphasizes client-centered care, connecting patients and their families with necessary healthcare and community resources to ensure timely access to appropriate services and continuity of care.
The responsibilities include:
- Conducting comprehensive assessments and developing tailored care plans.
- Coordinating services and advocating for patients’ needs.
- Providing education and support to patients and their families.
- Monitoring patient progress and ensuring accurate documentation.
Qualifications:
To qualify for this role, candidates must possess:
- A valid and active Registered Nurse license in the State of Florida.
- A minimum of three years of clinical experience, with at least one year in case management preferred.
Skills and Competencies:
Successful candidates will demonstrate:
- Proficiency in computer applications, particularly Excel, and familiarity with EMR systems.
- Strong organizational skills and the ability to manage multiple tasks effectively.
- Excellent communication and interpersonal skills.
