ACO Case Manager Job Description

Job Summary:

Millennium Physician Group is seeking a highly skilled ACO Case Manager to join our team. As a key member of our ACO team, you will be responsible for providing exceptional patient care and ensuring timely, coordinated access to medically appropriate levels of health and support services.

Key Responsibilities:

  • Assess patient needs and develop personalized care plans to achieve optimal health outcomes
  • Coordinate with healthcare providers, community resources, and family members to ensure seamless care transitions
  • Monitor patient progress, identify potential issues, and implement interventions to prevent complications
  • Document patient interactions, care plans, and outcomes in a timely and accurate manner
  • Collaborate with interdisciplinary teams to ensure comprehensive care and address patient concerns

Requirements:

  • Registered Nurse with active and clear license in the State of Florida
  • 3+ years of clinical experience with 1+ years of case management experience preferred
  • Strong computer skills, especially in Excel, and knowledge of EMR systems; Athena knowledge preferred but not required
