- Develop and implement patient care plans to meet individual needs
- Conduct regular assessments and evaluations to ensure patient safety and well-being
- Collaborate with healthcare providers to coordinate patient care and services
- Ensure compliance with regulatory requirements and company policies
- Current licensure to practice nursing in the practicing state
- Previous experience in home care setting with two years management or supervisory experience, preferred
- Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
- Competitive salary range: $75,000 - $80,000 per year
