Job Summary

Deliver high-quality hospice and palliative care to patients in their homes, nursing homes, or inpatient hospice settings. Develop and implement individualized care plans, collaborating with interdisciplinary teams to ensure seamless care coordination.

As a Hospice Nurse, you'll be responsible for:

  • Providing comprehensive assessments and developing individualized care plans
  • Delivering high-quality care to patients and their families

Requirements

To be successful in this role, you'll need:

  • A graduate degree in nursing from an accredited institution
  • Current licensure in the state of practice
  • Minimum 1 year of med/surg experience
  • Minimum 1 year of home care or hospice experience preferred
  • Ability to work flexible hours, including weekends, holidays, and night shifts
