Job Summary:

At Crossroads Hospice & Palliative Care, we are seeking a compassionate and skilled Nurse Practitioner to join our team. As a Hospice and Palliative Care Specialist, you will play a vital role in delivering high-quality patient care and providing emotional support to patients and their families.

Responsibilities:
  • Conduct patient evaluations and assessments in private residences, nursing facilities, and assisted living facilities.
  • Provide inpatient and outpatient palliative care consults, collaborating with attending physicians and medical directors.
  • Develop and implement individualized care plans that prioritize patient comfort, dignity, and quality of life.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care.
  • Participate in ongoing education and training to stay up-to-date on best practices in hospice and palliative care.

Requirements:
  • 2 years of experience as a Nurse Practitioner in an acute care environment.
  • Masters of Science in Nursing - Family Nurse Practitioner.
  • Active unencumbered state NP license.
  • Current driver's license and auto liability coverage.
  • DEA number and National Provider Identifier (NPI) required.
