Job Summary

We are seeking a skilled Rehab Liaison RN to join our team at Providence Home Infusion-Tukwila. As a Clinical Liaison RN, you will play a critical role in developing and maintaining home infusion referral relationships and coordinating home infusion services for patients from the University of Washington Health System Hospitals.

Key Responsibilities
  • Develop and maintain strong relationships with healthcare professionals and facilities to promote home infusion services
  • Coordinate and arrange home infusion services for referred patients, ensuring seamless transitions and high-quality care
  • Collaborate with interdisciplinary teams to provide comprehensive care and support to patients and families
  • Stay up-to-date with industry trends, best practices, and regulatory requirements to ensure compliance and excellence in care delivery
Requirements
  • 1 year of full-time experience in a clinical role
  • Bachelor's Degree in Nursing
  • Healthcare industry experience
  • Documented prior successful sales account medical management experience
  • Epic EMR experience
