Key Responsibilities- Collaborate with healthcare providers to optimize medication therapy and improve patient outcomes
- Conduct thorough medication reviews and provide counseling to patients and caregivers
- Develop and implement medication use evaluation programs to identify and mitigate medication-related problems
- Participate in multidisciplinary teams to coordinate patient care and improve health outcomes
- Stay up-to-date with the latest clinical guidelines and research to provide evidence-based care
- Doctorate of Pharmacy degree from an accredited institution
- 5 years of recent experience in clinical pharmacy practice
- Current licensure as a Pharmacist in the state of California
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
