Position Overview

We are looking for a dedicated Specialty Medicine Nurse Practitioner (NP) or Physician Assistant (PA) to enhance our healthcare team. This role emphasizes delivering exceptional patient care within a collaborative environment. The primary focus will be on providing continuity of care in the fields of pulmonology and sleep medicine, as well as conducting patient rounds in nursing homes and rehabilitation centers.

Key Responsibilities:

  • Assess and manage acute, chronic, or episodic health conditions.
  • Prescribe necessary medications and treatment plans.
  • Conduct thorough physical assessments of patients.
  • Formulate and execute patient management strategies.
  • Coordinate referrals to other healthcare providers and facilities.
