Care Coordinator, Utilization Management RN

Hackensack Meridian Health is seeking a skilled Care Coordinator, Utilization Management RN to join our team. As a key member of our healthcare team, you will be responsible for coordinating, communicating, and facilitating the clinical progression of patient treatment.
Key Responsibilities- Manage a designated patient caseload, planning effectively to reduce length of stay and promote efficient resource utilization.
- Collaborate with multidisciplinary teams to support length of stay reduction and observation management goals.
- Perform admission reviews, continued stay reviews, and termination of benefits, ensuring compliance with regulatory and payor requirements.
- Communicate with insurance companies, physicians, and other stakeholders to ensure seamless care coordination.
- Participate in quality improvement initiatives and identify areas for process improvement.
- BSN or BSN in progress, with a willingness to acquire within 3 years of hire.
- Effective decision-making skills, creativity in problem-solving, and influential leadership skills.
- Excellent verbal, written, and presentation skills.
- Moderate to expert computer skills, with proficiency in Epic, Xsolis Cortex, and Google Suites.
- Certified Case Manager (CCM), Certified Clinical Medical Assistant (CCMA), or American Case Management Association (ACMA) certification.
