Job Summary: As a per diem LPN, you will be responsible for delivering exceptional patient care, collaborating with interdisciplinary teams, and ensuring seamless transitions of care. Responsibilities:

  • Provide direct patient care, including vital signs, medication administration, and wound care
  • Collaborate with nurses, physicians, and other healthcare professionals to develop and implement individualized care plans
  • Conduct thorough assessments, documenting patient status and progress
  • Participate in interdisciplinary rounds, sharing knowledge and expertise to optimize patient outcomes
  • Maintain accurate records, adhering to facility policies and regulatory requirements

Requirements:

  • Current LPN license in the state of practice
  • Minimum 2 years recent experience as an LPN
  • BCLS certification
