Key Responsibilities:
  • Plan, coordinate, and deliver healthcare services and training programs to clients and caregivers.
  • Conduct initial home visits, supervisory visits, and coordinate client care plans.
  • Provide clinical teaching, competency validation, and staff training.
  • Ensure compliance with company policies, state licensure, and regulations.
  • Lead case conferences and other client-related meetings.
  • Facilitate communication between nursing staff, caregivers, family, and medical providers.
  • Directly oversee initial start of healthcare, assessment, and first home visit with ongoing performance monitoring.
  • Directly supervise and support the coordination, maintenance, evaluation, and termination of care plans and services.
Requirements:
  • At least three years of experience as a Licensed Practical Nurse with supervisory experience or a bachelor's degree in a related field.
  • Healthcare management experience with multiple age groups.
  • Minimum of three years of clinical experience.
  • Current and unrestricted license in the state of operation.
  • CPR professional certification.
  • Basic clinical skills, professional appearance, and demeanor.
  • Excellent oral, written, and interpersonal communication skills.
  • Excellent organizational and time management skills.
