About the Role

We are seeking a skilled Occupational Therapist, Physical Therapist, or Speech Therapist to join our team at Independence Home Health of the Broward. As a member of our home health care team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Job Summary

The successful candidate will administer occupational therapy to patients on an intermittent basis in their place of residence, in accordance with physicians' orders and plans of care under the direction and supervision of the Clinical Services Manager.

Responsibilities
  • Improves or minimizes residual physical disabilities of the patient.
  • Participates with all other home care personnel in patients' care planning.
  • Provides prescribed physical or occupational therapy.
  • Responsible for initial assessment, plan of care, maintenance program development and modifications, and reassessments every 30 days.
  • Consorts with physicians regarding changes in treatment.
  • Prepares clinical and progress notes.
  • Instructs patient/family members in home programs and fine motor movement exercises.
  • Helps develop the plan of care and revises as necessary.
  • Performs all skilled procedures.
  • Writes report to physician regarding the patient's progress.
Requirements
  • Graduate of a Physical and/or Occupational Therapy curriculum accredited by the American Medical Association.
  • Currently licensed and registered in the state practicing.
  • Two years of experience preferred, with home care experience a plus.

Work Environment

This position offers the opportunity to work in a dynamic healthcare environment, providing valuable services to patients in need of occupational therapy. The successful candidate will work indoors in our agency office and patient homes, traveling to/from patient homes as needed.
