Care Manager RN

The Team Based Care (TBC) Manager RN is a key member of our care team, responsible for providing high-quality, patient-centered care to a group of patients. This role requires strong clinical skills, excellent communication, and a passion for delivering exceptional patient experiences.
Key Responsibilities- Conduct shared patient visits with providers and independent RN visits using standardized procedures and protocols.
- Collaborate with other team members to coordinate and execute the plan of care.
- Demonstrate initiative in the delivery of patient care in a Team Based Care Model.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Graduate from an accredited school of Nursing.
- ~ 2 years of Professional Nursing Experience.
- Bachelor's Degree in Nursing.
- ~1 year of experience in ambulatory care nursing.
