Home Care Coordinator, LVN

High Desert PACE is seeking a skilled Licensed Vocational Nurse (LVN) to serve as a Home Care Coordinator. This role is responsible for coordinating homecare services for program participants, including the coordination of contracted home services, durable medical equipment, and nursing home care.
Key Responsibilities- Assess and Coordinate Care: Perform physical evaluations, including vital signs and blood glucose monitoring, and chart observations of participants' condition during every visit.
- Medication Management: Complete medication reconciliation and administer medication, screening tests, and immunizations as prescribed.
- Communication and Coordination: Coordinate and communicate with RNs, occupational therapists, physical therapists, physicians, social workers, dieticians, and others on the participants' care team.
- Documentation and Reporting: Document in accordance with PACE departmental guidelines and within the established time frame, and report changes in participants' condition to RN supervisors.
- Participant Support: Educate participants and their families on home care strategies and procedures, and listen to participants' questions, concerns, and requests.

- Active Valid Driver's License: Operate a motor vehicle for transport to home visits.
- Medical Equipment Operation: Operate medical equipment to assist with nursing care and provide appropriate treatment to participants.
- Work Environment: The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.
- Education and Training: Graduation from an accredited LVN school coupled with a current LVN license issued by the State of California Vocational Nursing and Psychiatric Technicians required.
- Experience: A minimum of one (1) year experience practicing as a Licensed Vocational Nurse, and at least one year working with a frail or elderly population.
- Certifications: Current CPR/BLS certification also required.
