The Care Manager RN is a key member of our team, responsible for providing high-quality patient care in a team-based care model. This role requires a strong nursing background, excellent communication skills, and the ability to work collaboratively with other healthcare professionals.

Key Responsibilities
  • Conduct shared patient visits with providers and independent RN visits using standardized procedures and protocols.
  • Collaborate with other team members to coordinate and execute the plan of care.
  • Use the nursing process to care for a group of patients safely, therapeutically, and efficiently.
  • Demonstrate initiative in the delivery of patient care and reflect the Providence core values.
Requirements
  • Graduate from an accredited school of Nursing.
  • California Registered Nurse License upon hire.
  • National Provider BLS American Heart Association upon hire.
  • 2 years of Professional Nursing Experience.
Preferred Qualifications
  • Bachelor's Degree in Nursing.
  • 1 year of experience in ambulatory care nursing.
