About the Role

Talent Doc is seeking a skilled Licensed Independent Social Worker (LISW) to join our team in providing exceptional mental health care to long-term care communities.

Key Responsibilities:

  • Provide counseling services to clients/residents in approximately 15 care facilities per month
  • Travel daily to facilities within a 60-mile radius, with varying facility types and acuity levels
  • Collaborate with a multidisciplinary team to deliver comprehensive care
  • Develop and implement individualized treatment plans
  • Document patient progress and maintain accurate records

Requirements:

  • LISW or LISW-S licensure in Ohio
  • Valid driver's license and reliable vehicle
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficient computer skills, with experience with Gerimed EMR a plus
