About the Job
We are seeking a compassionate and motivated Physical Therapist to join our team at JAG Physical Therapy. As a Physical Therapist, you will be responsible for assessing patients' needs and creating effective treatment plans to aid in their healing and recovery.
Responsibilities
- Maintain patient confidentiality and collaborate with the healthcare team to ensure high-quality patient care in a supportive environment.
- Create individualized treatment plans that address patients' unique needs and goals.
- Provide education and training to patients and their support systems on various topics such as ADLs/IADLs, therapeutic activities, and community reintegration.
- Conduct initial evaluations, daily notes, reassessments/progress reports, discharge plans, and communication with members of the team.
- Record daily billing and follow department guidelines for productivity.
- Select and administer tests and measures, synthesizing evaluation results to formulate diagnoses, goals, prognosis, and plans of care.
- Evaluate and re-evaluate the need for modifications to plans of care and goals, identifying opportunities for consultations or referrals to other healthcare professionals or services.
- Initiate and complete discharge plans, providing interventions to patients that are safe, effective, and in compliance with JAG's standards.
Requirements
- Bachelor's or Master's Degree required, Doctorate degree preferred.
- Licensed Physical Therapist in the state practicing in.
- Completed state-regulated CEU requirements in appropriate states (NY, NJ).
- Valid CPR License.
- Experience/knowledge with an EMR software is preferred.
- Excellent oral, written communication skills.
- Strong clinical decision-making skills.
- Excellent work ethic and dedication to patient success.
- Basic computer skills.
- Works as a team member and individually with minimal supervision.
