Position Overview:

  • Oversee patient care for individuals within the transplant population, acting as the main point of contact for both patients and referring healthcare providers throughout the transplant journey. Employ a collaborative team approach for patients with end-stage organ disease, leading to a tailored care plan that guides patients through various stages of transplant care. Ensure adherence to care plans and protocols that comply with multiple regulatory bodies, including CMS, OPTN/UNOS, and the Joint Commission.
Key Responsibilities:
  • Collect relevant medical data prior to clinic visits to anticipate patient needs regarding ongoing care and transplant management. Organize this information coherently and integrate it into the electronic medical record (EMR) for accessibility by all healthcare providers.
  • Utilize data-driven strategies to plan, coordinate, monitor, and document patient care throughout the solid organ transplant process within the EMR.
  • Present cases during care conferences, liaise with all stakeholders (patients, referring providers, consulting services, etc.), execute orders following established protocols, and review diagnostic test results, communicating findings to attending physicians as necessary.
  • Engage in local, regional, and national conferences related to transplant patient care or programmatic issues.
  • Complete essential documentation in accordance with Organ Procurement and Transplantation Network guidelines, including patient listings, status modifications, and post-transplant/donation follow-up data.
  • Maintain effective communication with external providers/agencies to support patients through various transitions of care.
  • Provide Financial Counselors with the necessary documentation for prior authorization related to transplantation.
  • Actively participate in the systematic review of protocols relevant to the transplant population.
  • Deliver comprehensive patient education that not only meets regulatory standards but also addresses the unique needs of each patient.
  • Serve as the primary resource for patients and their families throughout the transplant process.
  • Participate in multidisciplinary rounds and educational sessions within the department.
  • Educate fellow nurses and other multidisciplinary team members about patient care issues to foster positive outcomes for transplant patients. Monitor for deviations from the care plan and facilitate corrective measures as part of the quality improvement process.
  • Assist social workers in ensuring that adequate medical information is provided for entitlement and categorical program applications.
  • After a designated period, provide on-call coverage, which includes coordinating activities prior to transplant and serving as medical triage for outpatients in collaboration with the on-call physician. Manage the transplantation process when a donor is available, ensuring effective communication among physicians, local organ procurement organizations, patients/families, and the medical team involved.

  • Offer clinical supervision and evaluation of patient care related to the specialty area.
Work Schedule: 100%
Standard Hours: Monday to Friday, with on-call training commencing after a designated period, including weekends and holidays.
Location: University Campus
Compensation Grade: 56

Qualifications:
  • Baccalaureate Degree in Nursing is required.
  • 1-3 years of relevant nursing experience is necessary.
  • Strong written, verbal, and interpersonal communication skills are essential.
  • Experience with electronic health records is required.
  • Current Iowa Nursing License is mandatory.
Preferred Qualifications:
  • Demonstrated expertise in a specific clinical area of practice is preferred.
