Job Title: Community Education Prevention Initiative Birth to Three Home Visitor

This is a unique opportunity to join our team as a Home Visitor for the Community Education Prevention Initiative Birth to Three program. As a Home Visitor, you will play a vital role in providing comprehensive home-based early childhood services to pregnant moms and families with children birth to age three.

Key Responsibilities:
  • Implement the Baby Talk Curriculum within the home environment and socialization groups.
  • Stay current on and follow program policies and procedures manual.
  • Maintain a full caseload as assigned, typically 12 families.
  • Facilitate two home visits per month to assigned caseload, and complete 100% of home visits per year.
  • Implement developmentally appropriate activities and provide information that covers the eight key areas of instruction.
  • Assist parents in observing their child's developmental levels and enable them to serve as their child's first and most important teacher.
  • Provide parents with developmentally appropriate practices for the child's present level of development and provide activities of the next developmental level for each area of development emerging for the child.
  • Develop activity plans and home visits based on family's interests and child's development needs.
  • Conduct child developmental screenings and family assessments on a timely basis.
  • Monitor child's health record during home visits, and provide parents with information regarding childhood immunizations, and well-being as needed.
  • Assist families to develop a family individualized service plan during the enrollment period and biannually update.
  • Provide appropriate referrals to community services as needed and follow up as needed.
  • Coordinate activities and support family during transition plan.
  • Encourage parent participation in group encounters, parent workshops, and other program events.
  • Collaborate with other home visitors in planning group activities, attend, and participate in monthly group encounters with families to support the eight areas of instruction.
  • Develop an annual professional plan and participate in trainings, workshops, and professional opportunities provided by the Ounce of Prevention, Gateways to Opportunity, program supervisor, and other consultants.
  • Document and maintain individual records on children including home visits, screenings, assessments, referrals, and progress reports, as well as maintaining an electronic internal and external database on a timely basis as required.
  • Maintain and promptly submit accurate and complete electronic records and documentation as required by the program supervisor.
  • Complete annual paperwork for the PI grant requirements and the SIS data collection, such as 0-3 PI Parent Questionnaire, 0-3 PI Outcome Questionnaire, and PI Form by assigned deadlines.
  • Attend regularly scheduled monthly staff meetings and monthly individual reflective supervision.
  • Complete and submit calendar time log and mileage reimbursements monthly.
  • Work in collaboration with program supervisor and other staff members for the recruitment and enrollment of eligible families.
  • Participate in agency outreach and recruitment events.
  • Assist, plan, and coordinate bimonthly Parent Advisory Council meetings.
  • Perform other tasks as requested by the Prevention Initiative Supervisor or Program Supervisor.
Requirements:
  • Bachelor's degree in Childhood Education preferred or in the area of social work, human services, family studies, psychology, counseling, or related sciences required.
  • Must have or obtain CPR/AED and First Aid certificates.
  • Must have 2 years of experience in related field required.
  • Must submit to background check through District 214.
  • Must complete new employee GCN training and update annually as required according to deadlines.
  • Must complete the required BabyTalk training and others immediately following employment.
  • Must be able to work in a socially-and psychologically pressured environment.
  • Previous experience working with pregnant moms and children birth to age three.
  • Preference given to those with home visiting and/or group facilitation experience.
  • Preference given to those with Baby TALK training or similar model.
  • Must have proficiency in English and Spanish.
  • Must have excellent oral and written communication skills.
  • Must be able to effectively use technology related programs, such as Google Suite, Microsoft Suite, NewOrg or other data management systems. Home visitor will use an online database for all case management.
  • Must work independently and in cooperation with others.
  • Commitment to and passion for working with pregnant moms and children birth to age three.
  • Ability to take trainings and apply knowledge gained in education, child development, nutrition, child safety, mental health and other topics relevant for the position.
  • Must organize work flow to manage multiple responsibilities and deadlines concurrently, as well as set goals in a professional manner.
  • Must exercise discretion and good judgment clinically and ethically and to consult with supervisor as needed.
  • Must make home visits in the families' home (as Covid-19 restrictions allow).
  • Must establish trustworthy and respectful relationships with families to identify their strengths and support their needs.
  • Must be available to work an irregular schedule as necessary, including some evening and weekends.
  • Must have a valid driver's license to travel to work, family's homes and community organizations.
  • Must demonstrate appropriate interpersonal skills in dealing with staff, clients, students, parents, outside officials/agencies, and the general public.
  • Must project a positive image of the district as well as exhibit a positive employee attitude.
  • Must be capable of lifting a minimum of 20 pounds.
  • Must be able to perform frequent bending, stooping, and reaching activities.
