Job Description: Symptom Management Professional

We are AccentCare Medical Group, a leading provider of hospice and palliative care services. Our team of dedicated healthcare professionals is passionate about delivering compassionate and personalized care to individuals facing serious illness or end-of-life care.

About the Role:
We are seeking an experienced Symptom Management Professional to join our team. As a key member of our interdisciplinary team, you will play a vital role in managing symptoms and improving quality of life for our patients. Your expertise in goal-directed care and effective communication will be essential in delivering exceptional patient outcomes.

Responsibilities:
  • Assess and manage symptoms, including pain, anxiety, and shortness of breath.
  • Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans.
  • Provide education and support to patients and families on disease management and end-of-life care options.

Qualifications:

Maintains an active license and DEA in the state where care is being provided.


Salary Range:$130,000 - $160,000 per year (depending on experience)
