We are seeking a highly skilled Case Manager Registered Nurse (RN) to join our team. This role involves providing direct nursing care while working closely with the clinical team to create and implement personalized care plans that meet each patient's unique needs.

Key responsibilities include performing admission and concurrent reviews for all payers, maintaining accurate documentation, and coordinating unit activities. Additionally, you will apply established protocols to evaluate medical necessity and refer patients requiring extended care post-discharge to suitable long-term care facilities.

The ideal candidate will possess at least five years of recent clinical experience as an RN, with demonstrated leadership skills and certifications such as Basic Life Support (BLS). A Bachelor's Degree is preferred, but not required. The selected individual will be licensed as a Registered Nurse in the Commonwealth of Virginia or hold a Compact license.
