Job Summary and Qualifications

The Physical Therapist (PT) provides high-quality, patient-centered care by managing therapeutic care for an assigned group of patients in collaboration with physicians and the patient care team. This includes conducting patient assessments and participating in the development and implementation of individualized care plans aimed at achieving positive outcomes.

Responsibilities include:

  • Conducting evaluations and treatment interventions as per established professional practices.
  • Assessing therapeutic/rehabilitative/functional status and contributing to the overall care plan.
  • Delivering skilled therapeutic care in patients' homes using techniques aligned with the physician's plan.
  • Educating patients, families, and caregivers about therapies, exercises, and the use of therapeutic devices.
  • Coordinating and prioritizing caseloads, staffing needs, and assignment handoffs.

Qualifications:

  • Licensed Physical Therapist.
  • Valid Driver's License.
  • Bachelor's Degree.
