Job Summary:

We are seeking a highly skilled and dedicated PRN Registered Nurse to join our team of professionals who deliver high-quality patient care in the home setting.

Responsibilities:

  • Patient Assessment: Conduct thorough assessments of patients to identify physical, psychosocial, and environmental needs.
  • Care Planning: Develop individualized plans of care in collaboration with the interdisciplinary team to meet patients' unique needs.
  • Care Coordination: Coordinate and communicate effectively with patients, families, caregivers, and other healthcare professionals to ensure seamless care delivery.
  • Leadership and Supervision: Provide clinical direction and supervision to home health aides and LPNs/LVNs to ensure continuity and quality of care.

Qualifications:

  • BLS certification within 30 days of employment start date
  • Associate Degree or Registered Nurse Diploma
  • Valid driver's license
  • Registered Nurse licensure in good standing

Compensation: We offer a competitive salary range of $65,000 - $85,000 annually, depending on experience and qualifications. Estimated annual salary for this role is approximately $72,500.
