Position Overview

The Licensed Practical Nurse (LPN) operates under the supervision of a Registered Nurse or Nurse Manager, adhering to the LPN Scope of Practice. The LPN is dedicated to delivering patient care that encompasses age-appropriate interventions addressing physical, emotional, developmental, psychosocial, and educational needs. This role is crucial in ensuring the coordination and provision of high-quality healthcare services. Responsibilities include preparing patients for examinations, assisting during medical procedures, coordinating diagnostic tests and follow-up care, and documenting treatments in medical records. The LPN also plays a vital role in patient and family education, participates in staff meetings, in-services, special projects, and ongoing education. Additionally, they may serve as a mentor for newly hired LPN colleagues.

Key Responsibilities and Essential Functions

DIRECT PATIENT CARE
  • Evaluates and assists in managing the comprehensive needs of patients, following physician directives with RN oversight and employing the nursing process.
  • Executes routine nursing tasks to fulfill patient requirements under RN supervision.
  • Adheres to established productivity benchmarks.
  • Demonstrates proficiency in accessing and de-accessing ports, including flushing lines and maintaining sterile techniques.
  • Exhibits knowledge and skill in medication administration as defined within the scope of practice.
  • Seeks guidance, training, or supervision from leadership as necessary.
  • May assign tasks to unlicensed personnel, as appropriate and within their scope of practice.
  • Provides relationship-centered, patient-focused care that is sensitive to population-specific characteristics (e.g., age, gender, medical conditions).
  • Collaborates with patients, significant others, and/or family members to foster a comfortable and safe environment for care delivery, prioritizing the patient's goals in the care plan.
COORDINATION OF SERVICES FOR EPISODE OF CARE
  • Coordinates care and services with the RN and other healthcare team members.
  • Communicates changes in patient conditions to the RN, physician, midlevel provider, or clinical supervisor as appropriate.
  • Works with the RN to develop and adjust individual care plans based on patient needs and physician orders.
PATIENT / CAREGIVER EDUCATION
  • Acts as an educator for patients to promote self-care and caregiver support.
  • Utilizes teach-back methods to ensure understanding of self-care among patients and/or caregivers.
STAFF EDUCATION AND PROFESSIONAL DEVELOPMENT
  • Engages in the orientation and training of new staff and nursing students as appropriate.
  • Responsible for attending continuing education and in-service training as required.
  • Commits to maintaining current clinical knowledge relevant to patient care.
COMMUNICATION AND DOCUMENTATION
  • Ensures accurate and timely completion of required documentation.
  • Demonstrates understanding and application of regulatory guidelines.
  • Responsible for checking and responding to communications as required.
  • Regularly reviews work-related emails.
Required Minimum Education

Practical Nursing Diploma Required

Required Minimum License(s) and Certification(s)

All certifications are mandatory upon hire unless otherwise specified.

  • Basic Life Support (BLS) - Instructor

Required Minimum Experience:

Previous Healthcare Experience Preferred And

Experience in oncology, ambulatory, or outpatient settings preferred.
