About the Role

We are seeking a skilled Registered Nurse to join our Home Health team in Medford, OR. As a Home Health RN, you will work alongside an interdisciplinary team to provide high-quality care to patients in their homes.

Responsibilities
  • Provide comprehensive nursing care to patients in their homes, including assessments, care planning, and implementation.
  • Collaborate with the healthcare team to ensure seamless care transitions and coordination.
  • Develop and maintain strong relationships with patients and their families to ensure their needs are met.
  • Stay up-to-date with the latest clinical guidelines and best practices in home health care.
Requirements
  • Current Oregon Registered Nurse License.
  • 1 year of medical/surgical experience in an acute care setting.
  • 2 years of pediatric experience in an acute care setting for RN caring for pediatric patients.
  • National Provider BLS - American Heart Association within 30 days of hire.
