We are seeking a skilled Registered Nurse to join our Home Health team at Providence Medford Medical Center. As a Home Health RN, you will work alongside an interdisciplinary team to provide high-quality care to patients in the comfort of their own homes.
Responsibilities- Provide whole-person care to patients with a focus on their physical, emotional, and spiritual needs.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Administer medications and treatments as prescribed by physicians and other healthcare professionals.
- Current Oregon Registered Nurse License.
- National Provider BLS - American Heart Association certification.
- 1 year of medical-surgical experience in an acute care setting.
- 2 years of pediatric experience in an acute care setting for RN caring for pediatric patients.
