**Care Coordinator Opportunity**
Are you a skilled nurse looking to broaden your scope of experience in a dynamic and innovative environment? Cedars-Sinai is seeking a dedicated and compassionate Care Coordinator to join our team.
As a Care Coordinator, you will play a crucial role in charting the best trajectory of each patient's care, accommodating various needs, including medical, psycho-social, financial, and practical. You will coordinate the care and service of the patient population from admission through discharge, assessing and planning, implementing, monitoring, and evaluating options and services to meet an individual's health needs.
**Key Responsibilities:**
• Serve as a clinical co-pilot, coordinating patient care and services
• Assess and plan, implement, monitor, and evaluate options and services to meet patient needs
• Coordinate patient care and services from admission through discharge
**Requirements:**
• BSN preferred
• Current, unrestricted California RN license required
• American Heart Association BLS certification required
• Case Management Certification required within 2 years of hire
• A minimum of 3 years of acute care nursing experience required
• At least 3 years of Case Management experience preferred
